BACKGROUND: Cardiogenic shock (CS) is a significant complication of Takotsubo syndrome (TTS), contributing to heightened mortality and morbidity. Despite this, the Society for Cardiovascular Angiography and Interventions (SCAI) staging system for CS severity lacks validation in patients with TTS and CS. This study aimed to characterize a patient cohort with TTS using the SCAI staging system and assess its utility in cases of TTS complicated by CS. METHODS AND RESULTS: From a TTS national registry, 1591 consecutive patients were initially enrolled and stratified into 5 SCAI stages (A through E). Primary outcome was all-cause in-hospital mortality; secondary end points were TTS-related in-hospital complications and 1-year all-cause mortality. After exclusions, the final cohort comprised 1163 patients, mean age 71.0±11.8 years, and 87% were female. Patients were categorized across SCAI shock stages as follows: A 72.1%, B 12.2%, C 11.2%, D 2.7%, and E 1.8%. Significant variations in baseline demographics, comorbidities, clinical presentations, and in-hospital courses were observed across SCAI shock stages. After multivariable adjustment, each higher SCAI shock stage showed a significant association with increased in-hospital mortality (adjusted odds ratio: 1.77-29.31) compared with SCAI shock stage A. Higher SCAI shock stages were also associated with increased 1-year mortality. CONCLUSIONS: In a large multicenter patient cohort with TTS, the functional SCAI shock stage classification effectively stratified mortality risk, revealing a continuum of escalating shock severity with higher stages correlating with increased in-hospital mortality. This study highlights the applicability and prognostic value of the SCAI staging system in TTS-related CS.

INTRODUCTION: Endothelial dysfunction and platelet activation have been highlighted as possible mediators in Takotsubo syndrome (TTS). Nevertheless, to date, evidence on the usefulness of antiplatelet therapy in TTS remains controversial. The aim of our study is to evaluate long-term prognosis in TTS patients treated with antiplatelet therapy (APT) at hospitalization discharge. MATERIAL AND METHODS: An ambispective cohort study from the Spanish National Takotsubo Registry database was performed (June 2002 to March 2017). Patients were divided into two groups: those who received APT at hospital discharge (APT cohort) and those who did not (non-APT cohort). Primary endpoint was all-cause death. Secondary endpoints included the composite of recurrence or readmission and a composite of death, recurrence or readmission. RESULTS: From a total of 741 patients, 728 patients were alive at discharge. Follow-up was performed in 544 patients, who were included in the final analysis: 321 patients (59.0%) in the APT cohort and 223 patients (41.0%) in the non-APT cohort. The APT cohort had a better clinical presentation and received more heart failure and acute coronary syndrome-like therapies (angiotensin converting enzyme inhibitors/angiotensin receptor blockers: 75.1% vs. 51.1%; p<0.001, betablockers: 71.3% vs. 50.7%; p<0.001, statins: 67.9% vs. 33.2%; p<0.001). After adjusting for confounder factors, APT at discharge was a protective factor for all-cause death (adjusted hazard ratio (HR) 0.315, 95% confidence interval (CI): 0.106-0.943; p=0.039) and the composite endpoint of all-cause death, recurrence or readmission (adjusted HR 0.318, 95% CI: 0.164-0.619; p=0.001) at month 25 of follow-up. CONCLUSION: Patients with TTS receiving APT at discharge presented better prognosis up to two-years of follow-up compared with their counterparts not receiving APT.

OBJECTIVES: The SYNTAX score and SYNTAX score II have a high predictive capacity for adverse cardiovascular events. We aimed to demonstrate that both scores were good predictors of long-term adverse outcomes in an 'all-comers' population treated with a percutaneous coronary intervention (PCI). METHODS: In the study, we included 785 patients who received an angioplasty at our center between January 2011 and December 2012. The patients were distributed in tertiles according to the SYNTAX score and SYNTAX score II values; for the SYNTAX score - low ≤6.5 (n = 225); mid >6.5, <11.5 (n = 229); high ≥11.5 (n = 221); and for the SYNTAX score II PCI: low ≤20.5 (n = 226); mid >20.5, < 29.6 (n = 221); and high ≥29.6 (n = 218). RESULTS: The rates of major adverse cardiovascular events, death, cardiac death and new revascularizations at 3 years were significantly higher in the highest tertile of both the scores. For SYNTAX score: major adverse cardiovascular events, 12-15.3-21.7%, P < 0.001; death, 7.6-8.3-14%, P = 0.04; cardiac death, 3.2-2.7-7.5%, P = 0.03; new revascularizations, 4.5-8.6-10.4%, P = 0.001. For SYNTAX score II PCI: major adverse cardiovascular events, 8-10.9-28.9%, P < 0.001; death, 3.1-3.6-21.5%, P < 0.001; cardiac death, 0.9-0.5-11.4%, P < 0.001; new revascularizations, 4.5-8.2-11.3%, P = 0.03. CONCLUSION: The SYNTAX score II showed better predictive capacity than the SYNTAX score for major adverse cardiovascular events, death and cardiac death, with no difference noted for new revascularizations, and it was an independent predictor for these events in an 'all-comers' population.

ANTECEDENTES Y OBJETIVO: Se ha postulado una relación entre neoplasias y síndrome de Takotsubo (STB). Nuestro objetivo fue determinar su prevalencia en enfermos con STB, comparar el perfil clínico de STB con cáncer o no y estudiar su evolución a largo plazo. MATERIALES Y MÉTODOS: Analizamos la situación oncológica de pacientes incluidos en el REgistro nacional sobre síndrome de TAKOtsubo (RETAKO), ingresados entre 2002 y 2019, aportados por 38 hospitales de todo el país. Se admitió cualquier antecedente de malignidad/tumor que, aun benigno, recibiera quimioterapia, radioterapia o cirugía específica, actual o pasado. RESULTADOS: Se describió cualquier tipo de neoplasia en 129 (11,8%) casos, dentro de una cohorte de 1.097 pacientes con STB. Los oncológicos, con menor porcentaje de mujeres (79,1 vs. 88,3%; p = 0,003), mostraron una tendencia no significativa a mayor edad, hiperuricemia, apnea del sueño, con un menor FEVI al ingreso, y un perfil de factores de riesgo cardiovascular similar, pero más anemia crónica e inmunosupresión. La neoplasia más frecuente fue el cáncer de mama. Durante su estancia hospitalaria, los oncológicos mostraron más complicaciones, destacando insuficiencia cardiaca/shock, insuficiencia renal aguda y una tendencia en el combinado de infecciones. En el seguimiento, presentaron mayor mortalidad y más sucesos combinados MACE, con una tendencia, no significativa, a la aparición de recurrencias o reingresos cardiovasculares. CONCLUSIONES: La prevalencia de neoplasias en pacientes con STB fue elevada. La presentación clínica fue diferente en relación con los pacientes que no las presentan y suponen probablemente un factor de riesgo de peor pronóstico, tanto hospitalario como a largo plazo

On follow-up, they presented higher mortality and more combined MACE events, with a non-significant trend in the occurrence of cardiovascular recurrences or readmissions. CONCLUSIONS: The prevalence of neoplasms in patients with TS is high. The clinical presentation is different in relation to patients who do not have neoplasms and they probably represent a risk factor for a worse hospital and long term prognosis

BACKGROUND: Coronary Aneurysms are a focal dilatation of an artery segment >1.5-fold the normal size of adjacent segments. Although some series have suggested a prevalence of 0.3-12%, data are lacking. In addition, they are not mentioned in practice guidelines. Our aim was investigate its prevalence, management and long-term outcomes. METHODS AND RESULTS: The coronary artery aneurysm registry (CAAR) involved 32 hospitals across 9 countries in America and Europe. We reviewed 436,467 consecutive angiograms performed over the period 2004-2016. Finally, 1565 patients were recruited. Aneurysm global prevalence was 0.35%. Most patients were male (78.5%) with a mean age of 65 years and frequent cardiovascular risk factors. The main indication for angiogram was an acute coronary syndrome, 966 cases. The number of aneurisms was ≤2 per patient in 95.8% of the cases, mostly saccular, most frequently found in the left anterior descending and with numbers proportional with coronary stenosis. Aortopathies were related with more aneurysms too. Most patients received any revascularization procedure (69%), commonly percutaneous (53%). After a median follow-up of 37.2 months, 485 suffered a combined event (MACE) and 240 died. Without major differences comparing CABG vs PCI, MACE and death were more frequent in patients who received bare metal stents. CONCLUSIONS: Coronary artery aneurysms are not uncommon. Usually, they are associated with coronary stenosis and high cardiovascular risk. Antiplatelet therapy seems reasonable and a percutaneous approach is safe and effective.

Background Takotsubo syndrome (TTS) is an acute reversible heart condition initially believed to represent a benign pathology attributable to its self-limiting clinical course; however, little is known about its prognosis based on different triggers. This study compared short- and long-term outcomes between TTS based on different triggers, focusing on various physical triggering events. Methods and Results We analyzed patients with a definitive TTS diagnosis recruited for the Spanish National Registry on TTS (RETAKO [Registry on Takotsubo Syndrome]). Short- and long-term outcomes were compared between different groups according to triggering factors. A total of 939 patients were included. An emotional trigger was detected in 340 patients (36.2%), a physical trigger in 293 patients (31.2%), and none could be identified in 306 patients (32.6%). The main physical triggers observed were infections (30.7%), followed by surgical procedures (22.5%), physical activities (18.4%), episodes of severe hypoxia (18.4%), and neurological events (9.9%). TTS triggered by physical factors showed higher mortality in the short and long term, and within this group, patients whose physical trigger was hypoxia were those who had a worse prognosis, in addition to being triggered by physical factors, including age >70 years, diabetes mellitus, left ventricular eyection fraction <30% and shock on admission, and increased long-term mortality risk. Conclusions TTS triggered by physical factors could present a worse prognosis in terms of mortality. Under the TTS label, there could be as yet undiscovered very different clinical profiles, whose differentiation could lead to individual better management, and therefore the perception of TTS as having a benign prognosis should be generally ruled out.

OBJECTIVES: This study sought to describe the incidence, determinants, and prognostic impact of cardiogenic shock (CS) in takotsubo syndrome (TTS). BACKGROUND: TTS can be associated with severe hemodynamic instability. The prognostic implication of CS has not been well characterized in large studies of TTS. METHODS: We analyzed patients with a definitive TTS diagnosis (modified Mayo criteria) who were recruited for the National RETAKO (Registry on Takotsubo Syndrome) trial from 2003 to 2016. Cox and competing risk regression models were used to identify factors associated with mortality and recurrences. RESULTS: A total of 711 patients were included, 81 (11.4%) of whom developed CS. Male sex, QTc interval prolongation, lower left ventricular ejection fraction at admission, physical triggers, and presence of "a significant" left intraventricular pressure gradient, were associated with CS (C index = 0.85). In-hospital complication rates, including mortality, were significantly higher in patients with CS. Over a median follow-up of 284 days (interquartile range: 94 to 929 days), CS was the strongest independent predictor of long-term, all-cause mortality (hazard ratio [HR]: 5.38; 95% confidence interval [CI]: 2.60 to 8.38); cardiovascular (CV) death (sub-HR: 4.29; 95% CI: 2.40 to 21.2), and non-CV death (sub-HR: 3.34; 95% CI: 1.70 to 6.53), whereas no significant difference in the recurrence rate was observed between groups (sub-HR: 0.76; 95% CI: 0.10 to 5.95). Among patients with CS, those who received beta-blockers at hospital discharge experienced lower 1-year mortality compared with those who did not receive a beta-blocker (HR: 0.52; 95% CI: 0.44 to 0.79; pinteraction = 0.043). CONCLUSIONS: CS is not uncommon and is associated with worse short- and long-term prognosis in TTS. CS complicating TTS may constitute a marker of underlying disease severity and could identify a masked heart failure phenotype with increased vulnerability to catecholamine-mediated myocardial stunning.

Introducción y objetivos: La angiografía coronaria rotacional (ACR) permite reducir la cantidad de contraste administrado y puede prevenir el desarrollo de nefropatía inducida por contraste (NIC) durante los procedimientos coronarios invasivos. El objetivo del estudio es evaluar el impacto de la ACR en la aparición de NIC (aumento de creatinina ≥0,5mg/dL o ≥25%) tras un síndrome coronario agudo. Métodos: De abril a septiembre de 2016 se seleccionaron prospectivamente pacientes con síndrome coronario agudo remitidos para coronariografía diagnóstica con posibilidad de angioplastia ad hoc, que fueron estudiados con ACR o angiografía coronaria convencional (ACC) según criterio del operador. Se compararon la NIC (variable de valoración primaria), variables analíticas, angiográficas y clínicas. Resultados: De 235 pacientes reclutados, 116 pacientes fueron estudiados con ACR y 119 pacientes con ACC. El grupo de ACR presentaba mayor edad (64,0±11,8 vs. 59,7±12,1 años; p=0,006), más mujeres (44,8 vs. 17,6%; p<0,001) y peor filtrado glomerular estimado (76±25 vs. 86±27 mL/min/1,73 m2; p=0,001), con menos angioplastias (p<0,001). Asimismo, el grupo de ACR recibió menos contraste (113±92 vs. 169±103 mL; p<0,001), diferencias que se mantuvieron en los procedimientos diagnósticos (54±24 vs. 85±56 mL; p<0,001) y diagnóstico-terapéuticos (174±64 vs. 205±98 mL; p=0,049). El grupo de ACR presentó menos NIC (4,3 vs. 22,7%; p<0,001): en el análisis de regresión se objetivó que continuaba relacionándose con menor desarrollo de NIC (riesgo relativo ajustado: 0,868; IC 95%: 0,794-0,949; p=0,002). No hubo diferencias en las variables clínicas. Conclusiones: La ACR se asoció con menor administración de contraste durante procedimientos coronarios invasivos tras un síndrome coronario agudo, lo que resultó en una menor aparición de NIC (AU)


Introduction and objectives: Rotational coronary angiography (RCA) requires less contrast to be administered and can prevent the onset of contrast-induced nephropathy (CIN) during invasive coronary procedures. The aim of the study is to evaluate the impact of RCA on CIN (increase in serum creatinine ≥0.5mg/dl or ≥25%) after an acute coronary syndrome. Methods: From April to September 2016, patients suffering acute coronary syndromes who underwent diagnostic coronary angiography, with the possibility of ad hoc coronary angioplasty, were prospectively enrolled. At the operator's discretion, patients underwent RCA or conventional coronary angiography (CCA). CIN (primary endpoint), as well as analytical, angiographic and clinical endpoints, were compared between groups. Results: Of the 235 patients enrolled, 116 patients received RCA and 119 patients received CCA. The RCA group was composed of older patients (64.0±11.8 years vs. 59.7±12.1 years; p=0.006), a higher proportion of women (44.8 vs. 17.6%; p<0.001), patients with a lower estimated glomerular filtration rate (76±25 vs. 86±27ml/min/1.73 m2; p=0.001), and patients who underwent fewer coronary angioplasties (p<0.001) compared with the CCA group. Furthermore, the RCA group, received less contrast (113±92 vs. 169±103ml; p<0.001), including in diagnostic procedures (54±24 vs. 85±56ml; p<0.001) and diagnostic-therapeutic procedures (174±64 vs. 205±98ml; p=0.049) compared with the CCA group. The RCA group presented less CIN (4.3 vs. 22.7%; p<0.001) compared to the CCA group, and this finding was maintained in the regression analysis (Adjusted relative risk: 0.868; 95% CI: 0.794-0.949; p=0.002). There were no differences in clinical endpoints between the groups. Conclusions: RCA was associated with lower administration of contrast during invasive coronary procedures in acute coronary syndrome patients, resulting in lower incidence of CIN, in comparison with CCA (AU)

OBJECTIVE: During cardiology training, the cardiology fellow has to be trained in all things related to the indication, interpretation, and performing of nuclear cardiology studies using single photon emission computed tomography (SPECT). The aim of the present study was to analyse the relationship between the adequacy of indications of myocardial perfusion gated-SPECT and the years of experience since the completion of cardiology training. METHOD: A descriptive, retrospective analysis was performed on a single-centre register, in which the indications (adequate or inadequate use) were recorded according to myocardial perfusion gated-SPECT guidelines, prescribed by cardiologists of a university hospital. RESULTS: A total of 950 gated-SPECT tests were analysed according to the appropriate or inadequate indication. The sample of study was distributed in quartiles (years) since the cardiologist finished the residency. Cardiologists with less than 10 years of clinical experience reported a higher proportion of gated-SPECT tests compared to the more experienced cardiologists (87.6 vs. 9.3%, P<.001). After adjusting for age, gender, and cardiovascular risk factors, the multivariate analysis showed that, for each year of experience after completion of cardiology training, the probability of adequately indicating the test (OR: 1.33, 95% CI: 1.29-1.38, P<.001) was statistically significant. CONCLUSIONS: The professional experience of the clinical cardiologist is the most important factor to perform an appropriate indication of gated-SPECT myocardial perfusion.

INTRODUCTION AND OBJECTIVES: Rotational coronary angiography (RCA) requires less contrast to be administered and can prevent the onset of contrast-induced nephropathy (CIN) during invasive coronary procedures. The aim of the study is to evaluate the impact of RCA on CIN (increase in serum creatinine ≥0.5mg/dl or ≥25%) after an acute coronary syndrome. METHODS: From April to September 2016, patients suffering acute coronary syndromes who underwent diagnostic coronary angiography, with the possibility of ad hoc coronary angioplasty, were prospectively enrolled. At the operator's discretion, patients underwent RCA or conventional coronary angiography (CCA). CIN (primary endpoint), as well as analytical, angiographic and clinical endpoints, were compared between groups. RESULTS: Of the 235 patients enrolled, 116 patients received RCA and 119 patients received CCA. The RCA group was composed of older patients (64.0±11.8 years vs. 59.7±12.1 years; p=0.006), a higher proportion of women (44.8 vs. 17.6%; p<0.001), patients with a lower estimated glomerular filtration rate (76±25 vs. 86±27ml/min/1.73 m2; p=0.001), and patients who underwent fewer coronary angioplasties (p<0.001) compared with the CCA group. Furthermore, the RCA group, received less contrast (113±92 vs. 169±103ml; p<0.001), including in diagnostic procedures (54±24 vs. 85±56ml; p<0.001) and diagnostic-therapeutic procedures (174±64 vs. 205±98ml; p=0.049) compared with the CCA group. The RCA group presented less CIN (4.3 vs. 22.7%; p<0.001) compared to the CCA group, and this finding was maintained in the regression analysis (Adjusted relative risk: 0.868; 95% CI: 0.794-0.949; p=0.002). There were no differences in clinical endpoints between the groups. CONCLUSIONS: RCA was associated with lower administration of contrast during invasive coronary procedures in acute coronary syndrome patients, resulting in lower incidence of CIN, in comparison with CCA.

Resumen Objetivo: Durante el período de formación, el residente de cardiología ha de entrenarse en todo lo relacionado con la indicación, la interpretación y realización de imágenes en cardiología nuclear que usan la tomografía computarizada por emisión de fotón simple (SPECT). El objetivo del presente estudio fue analizar la relación existente entre la adecuación de las indicaciones del gated-SPECT de perfusión miocárdica y los años de experiencia desde la finalización de la residencia de cardiología. Método: Registro descriptivo, retrospectivo y unicéntrico, en el que se analizaron las indicaciones (uso adecuado e inadecuado), según las guías de la gated-SPECT de perfusión miocárdica, prescritas por cardiólogos de un hospital universitario. Resultados: Se analizaron un total de 950 pruebas de gated-SPECT de acuerdo a la indicación adecuada e inadecuada y se distribuyeron por cuartiles los años de finalización de la residencia del cardiólogo prescriptor. Los cardiólogos de menos de 10 años de experiencia clínica indican de forma inadecuada una mayor proporción de pruebas de gated-SPECT que los cardiólogos de mayor experiencia (87,6 vs. 9,3%: p < 0,001). Tras ajustar por edad, sexo y factores de riesgo cardiovascular, el análisis multivariante mostró que por cada año de experiencia tras finalizar la residencia de cardiología aumenta un 33% la probabilidad de indicar de forma adecuada la prueba (OR: 1,33; IC 95%: 1,29-1,38; p < 0,001). Conclusiones: La experiencia profesional del cardiólogo clínico es el factor más importante para realizar una indicación adecuada de la gated-SPECT de perfusión miocárdica.


Abstract Objective: During cardiology training, the cardiology fellow has to be trained in all things related to the indication, interpretation, and performing of nuclear cardiology studies using single photon emission computed tomography (SPECT). The aim of the present study was to analyse the relationship between the adequacy of indications of myocardial perfusion gated-SPECT and the years of experience since the completion of cardiology training. Method: A descriptive, retrospective analysis was performed on a single-centre register, in which the indications (adequate or inadequate use) were recorded according to myocardial perfusion gated-SPECT guidelines, prescribed by cardiologists of a university hospital. Results: A total of 950 gated-SPECT tests were analysed according to the appropriate or inade- quate indication. The sample of study was distributed in quartiles (years) since the cardiologist finished the residency. Cardiologists with less than 10 years of clinical experience reported a higher proportion of gated-SPECT tests compared to the more experienced cardiologists (87.6 vs. 9.3%, P < .001). After adjusting for age, gender, and cardiovascular risk factors, the multiva- riate analysis showed that, for each year of experience after completion of cardiology training, the probability of adequately indicating the test (OR: 1.33, 95% CI: 1.29-1.38, P < .001) was statistically significant. Conclusions: The professional experience of the clinical cardiologist is the most important factor to perform an appropriate indication of gated-SPECT myocardial perfusion.

Coronary artery aneurysm is defined as a coronary dilation that exceeds the diameter of adjacent segments or the diameter of the patient's largest normal coronary vessel by 1.5×. It is an uncommon disease that has been diagnosed with increasing frequency since the widespread appearance of coronary angiography. The published incidence varies from 1.5% to 5%, suggesting male dominance and a predilection for the right coronary artery. Although several causes have been described, atherosclerosis accounts for ≥50% of coronary aneurysms in adults. Reported complications include thrombosis and distal embolization, rupture, and vasospasm, causing ischemia, heart failure, or arrhythmias. The natural history and prognosis remain unknown, as definitive data are scarce. Controversies persist regarding the use of medical management (antithrombotic therapy) or interventional/surgical procedures. Only some case reports or small case series are available about this condition. The Coronary Artery Aneurysm Registry (CAAR; http://www.ClinicalTrials.gov NCT02563626) is a multicenter international ambispective registry that aims to provide insights on anatomic, epidemiologic, and clinical aspects of this substantially unknown entity. In addition, the registry will assess management strategies (conservative, interventional, or surgical) and their short- and long-term results in a large cohort of patients. BACKGROUND: Takotsubo syndrome (TKS) usually mimics an acute coronary syndrome. However, several clinical forms have been reported. Our aim was to assess if different stressful triggers had prognostic influence on TKS, and to establish a working classification. METHODS: We performed an analysis including patients with TKS between 2003-2013 from our prospective local database and the RETAKO National Registry, fulfilling Mayo criteria. Patients were divided in two groups regarding their potential triggers: (a) none/psychic stress as 'primary forms' and (b) physical factors (asthma, surgery, trauma, etc.) as 'secondary forms'. RESULTS: Finally, 328 patients were included, 90.2% women, with a mean age of 69.7 years. Patients were divided into primary TKS (n=265) and 63 secondary TKS groups. Age, gender, previous functional class and cardiovascular risk profile displayed no differences between groups before admission. However, primary-TKS patients suffered a main complaint of chest pain (89.4% vs 50.7%, p<0.0001) with frequent vegetative symptoms. Regarding treatment before admission, there were no differences either. During admission, differences were related to more intensive antithrombotic and anxiolytic drug use in the primary TKS group. Inotropic and mechanical ventilation use was higher in the secondary cohort. After discharge, a more frequent prescription of beta-blockers and statins in primary-TKS patients was seen. Secondary forms displayed more in-hospital stay and evolutive complications: death (hazard ratio (HR): 3.41; 95% confidence interval (CI): 1.14-10.16, p=0.02), combined event variable (MACE) (HR: 1.61; 95% CI: 1.01-2.6, p=0.04) and recurrences (HR: 1.85; 95% CI: 1.06-3.22, p=0.02). CONCLUSION: Secondary TKS could present or mark worse short and long-term prognoses in terms of mortality, recurrences and readmissions. We propose a simple working nomenclature for TKS.

Introducción y objetivos: El síndrome de tako-tsubo es una entidad de etiología y epidemiología inciertas, capaz de semejarse a un infarto y que, aunque suele tener buen pronóstico, no está exenta de complicaciones. El objetivo del presente trabajo es caracterizar esta enfermedad en nuestro medio a través de un registro (REgistro nacional multicéntrico sobre síndome de TAKOtsubo). Métodos: El registro prospectivo incluyó a 202 pacientes incidentes en 23 hospitales entre 2012-2013. Se recogieron las características clínicas, analíticas, electrocardiográficas y de imagen, así como los eventos durante el seguimiento. Se incluyó a los pacientes cuando sus médicos consideraron probado el caso, y la incidencia de la enfermedad se calculó en función de los cateterismos solicitados con el diagnóstico presunto de síndrome coronario agudo. Resultados: Los casos fueron predominantemente mujeres (90%), con una media de edad de 70 años y frecuentes factores de riesgo cardiovascular, hipertensión arterial (67%), dislipemia (41%), diabetes mellitus (15%) y tabaquismo (15%). Sin clara distribución semanal o estacional, se calculó una incidencia del 1,2%. El dolor torácico fue el síntoma predominante, con algún desencadenante (psíquico, físico o ambos) en el 72%, y la mayoría consultó en las primeras 6 h. La estancia mediana fue de 7 días; apareció algún síntoma de insuficiencia cardiaca en el 34,0% y arritmias en el 26,7% y murió el 2,4%.ConclusionesEl síndrome de tako-tsubo es una enfermedad poco incidente, que afecta predominantemente a mujeres posmenopáusicas, en más de la mitad de los casos tras una situación psicológicamente estresante. Se caracteriza por dolor anginoso, sin distribución estacional; aunque no está exenta de morbimortalidad, conlleva buen pronóstico (AU)
